As directed by the Tennessee Department of Health (TDOH), all children entering the 7th grade are required to receive and show proof of a tetanus-diphtheria-pertussis booster (Tdap).
This immunization must be recorded on the official Tennessee immunization form. Please provide the school with the original form or a good copy, as faxes are often hard to read. All 7th grade students as of 2026-2027 must present the new immunization form on or before the first day of school.
